   APOE4 is what I think you are calling the brain bleed gene. It does not predict that you will get Alzheimer's only a risk factor for getting it....50% risk. That is not a strong enough correlation for me to get the genetic test personally. Doctors will usually test for this to decide if someone is a good candidate for the amyloid plaque clearing drugs so your insurance might not cover it. My sister's don't agree with me though and are going to pay out of pocket because they think why wouldn't I want to know. There are whole populations of people who have this gene and they don't have Alzheimer's. Until they find a literal gene that causes Alzheimer's, I will do my best with lifestyle but everything else is out of my control. If you are having memory issues then that's a different story.

   Wow, from every single person on this thread. We hear you and we feel your pain. First, please download the book The 36 Hour Day. It will become a daily go to and help with so many issues. Next, what is your wife's diagnosis? Dementia or Alzheimers? Then, which sub group. There are five major sub groups and over 100 less common groups. It is important to know which one she has and which part of her brain is affected. For example, Lewy Body Dementia is a completely different part of the brain than Frontotemporal Dementia, and both of them are not Alzheimers. They are treated similarly but not the same. My own personal opinion dealing with my husband, 75, with Alzheimers APOE 3/ APOE 4, is we sold our house of 20 years, moved to a condo that handles all outside care, and has a pool. Downsizing basically by myself was so traumatic, and I am glad that is behind us. If I had it to do over, i would have sent him to stay a few days with his brother and moved with him gone. He could not help, follow directions, and just kept asking the same questions over and over. He likes the condo, has a small patio, and loves walking down to swim in the pool. Being close to family is a must. I was afraid he would want to go back to the house, and he has not even mentioned the other house even once. Go figure. Get the book, and use her diagnosis to understand your journey.

   Early Symptoms - My LO just turned 60 and in the last several months I have noticed things that I believe need to be talked about with their doctor. Recently they have forgot conversations that we had the previous day and since they have been forgetful in the past I have just shrugged it off. They have also started mixing up peoples names ( usually say the dads name instead of the son ) and there have been two instances where we have talked about plans in person, but then the plans were changed in their mind and they have either met us at different times or blended where the location would be. Their mother passed from Alzheimer’s and per 23andme have the APOE 4. Should I be concerned or is this just sounding like normal forgetfulness?

   Have you asked your wife? As an AD patient myself, I tend to notice how infrequently the patient is consulted. How does she feel about more doctors appointments, frequent MRIs and getting IV infusions? Would she prefer to have more comforting, reliable home routine than the disruptions that come with some of the medication protocols? My mom had a great experience with donezepil. It gave her two solid years of stability before her AD progression began again. However, that was a decision I made for her. Knowing what I know now, I wish I'd included her in the decision although I think we would have reached the same conclusion. For myself, I have signed up for the full complement of life disruptions in a clinical trial...office appointments, monthly MRIs, side effects (I'm heterozygous APOE4.) and hours of cognitive testing. The testing is by far the most disruptive and anxiety inducing aspect for me. The infusions are subcutaneous and I have recently begun to do them myself at home. I would probably have resented all the commitments involved here if I hadn't been involved with the decision making. OP, you are right that there is a lot to consider while swimming in a sea of unknowns. Don't forget to include your mom's feelings. They are still valid. She's not gone yet.

   So you might get an additional 6 months at this stage as a best case scenario? It won't get better. The decline is still going to happen. There will be no improvement in symptoms. It's a slow burn but still a burn. My Mom made it through 2 months of Kisunla before getting severe brain bleeds and brain swelling. She does not have two copies of APOE4. We stopped treatment of course and we all noticed a big decline since. Wish we would have never done it. It's like she never recovered from the ARIA. But when there are no other choices you feel like you have to try it. If your neurologist says you are not a good candidate, believe them.
