First let me say how sorry I am that you and your wife are going through this. A couple of suggestions: Regarding neurologist appointments. The doctor can help with medications. What meds will work changes as the disease progresses and also varies from person to person. My husband’s neurologist also had a social worker from Alzheimer’s Association at the appointments to let me know of resources etc. Regarding assisted living/memory care etc. You need to tour them but really your wife does not. If the time comes that you feel she needs placement she will not recall if she visited that facility before. This is a horrible disease.

My opinion, based on everything I've read, is that no single solution/treatment has shown to cure or stop the progression of AD because the origin is multi-faceted, and I'm believing the cause starts outside the brain. There is evidence to back that up. That said, my wife (55) was diagnosed with EOA a year ago. Her symptoms started before that. Her neurologist denied her the amyloid treatment because she believed it was too late and would not benefit my wife. The tests reveal no genetic cause for her condition. So, I'm left to look elsewhere. The doctor is only going to treat symptoms at this point. I firmly believe two major contributions to her condition have been poor sleep habits, and her picky diet (ie. the gut-brain axis). All my efforts have been to diminish those negative influences. So, outside of the usual donepezil and memantine, my wife is on hormone replacement therapy (estradiol patch and progesterone), and I have her on a slew of supplements, we go on near-daily walks, I have her at the local gym twice a week for workouts (including weight training), and have altered her diet. My best description of her condition is "stable", based on my observations. But, I've also seen a couple of things (very recently) that give me some encouragement. She seems more alert, and is putting on makeup (something she hasn't paid attention to in awhile). Whether I'm seeing things I want to see, and if it's related to everything we're doing, or the fact that I increased her Lithium Orotate dose a couple of weeks ago, I can't say for sure. It could be all of the above.

Thats awesome, I give my dad usually 6-9mg of melatonin as well. I need to get a better neurologist, maybe I need to go to the hospital ER instead. The current neurologist just prescribed seroquel, and then I had to beg for a dose increase and then she said nothing else can be done until she further evaluates my dad in 4 weeks. I guess she doesnt want to be liable for anything, but the doctors at big hospitals seem much smarter and might give me the meds that are needed. Things will get worse over time, if you want to help or be there for your parents you might have to consider moving to their city. Especially if they dont have much family nearby to them. The way parent A is acting sounds like it will be a nightmare when it gets worse. Someone needs to really talk with them, and calm them down and make them understand that they are getting signs of dementia and they need to speak with the doctor. Parent B definitely needs to step away during that time, maybe for a week. Parent A will not listen to Parent B, someone else like yourself needs to go over there and handle your parent. If he has any siblings that he listens to that would be another option. Maybe 2 family members together can go with him to the doc. Best of luck. 🙏

If you can find an integrated practice that includes advanced drug therapy, geriactrics, and access to a specialized social worker and counselor that's the way to go. I was lucky enough to get my mom seen by the doctor who co-wrote the book "Take Your Oxygen First," which is aimed at caregivers. She also helped create this particular integrated model for care. I suspect that practice and others like it have more opportunities to participate in multiple trials of promising new therapies. I have to say I really admire your courage in seeking an early diagnosis. Between my family history and other factors, I am pretty sure I will start to show symptoms by the time I'm retirement age. But I just don't want to know. I know that's stupid but there you have it.
