    My LO (F83) has been on Kisunla for 18 months and it was a success for her. No side effects other than some fatigue. No microbleeds or headaches. The PET scan showed a reduction in plaque. Ultimately, I think it bought us all some time and has slowed the inevitable progression. Everyone needs to make their own decision on this.

    Yes, I remember that it was tough soon after an MCI diagnosis. I'm curious as to why you didn't mention one of the new anti-amyloid treatments (Leqembi & Kisunla)? I was diagnosed in Jan 2024 and started on Leqembi in June 2024. Leqembi was the only treatment readily available at that time. I recently had infusion #48 (I'm now on what is referred to as maintenance dosing). My cognition seems to be holding steady. There are dangers with these drugs (brain swelling and bleeding), so you have to weight the cost vs benefits.

    So you might get an additional 6 months at this stage as a best case scenario? It won't get better. The decline is still going to happen. There will be no improvement in symptoms. It's a slow burn but still a burn. My Mom made it through 2 months of Kisunla before getting severe brain bleeds and brain swelling. She does not have two copies of APOE4. We stopped treatment of course and we all noticed a big decline since. Wish we would have never done it. It's like she never recovered from the ARIA. But when there are no other choices you feel like you have to try it. If your neurologist says you are not a good candidate, believe them.

    "Getting angry" usually referred to as agitation doesn't affect everyone. If that's the way your doctor thinks of it and refers to it, I would have some concerns about being over medicated as a control mechanism. Is your treatment and care being managed by a neurologist? If not, I recommend finding one with expertise in AD. I empathize with you. I received an early AD diagnosis about 15 months ago. I live alone and have no family. I am honestly scared. Managing my anxiety is a challenging part of my self-care and maintaining optimal functionality. Did you have a Ptau blood test or a PET scan? Personally, I would insist on an objective data-based diagnosis. Don't settle for a subjective diagnosis based solely on cognitive testing and interpretation. Do you know if you carry the APOE4 gene? My mom experienced almost exactly 2 years of stability and no side effects with donepezil. After 2 years, the disease progression returned with what seemed like a new intensity. From what I've read, this is fairly typical. However, there are some newer medications that can provide better results. Have you considered trying Kisunla? It's an infusion for people with early diagnosis. It clears amyloid plaque from your blood vessels. This stops the progression and stabilizes your condition for a longer period than donepezil. There's so much to consider once there's been a solid data-based diagnosis. You are fortunate to have a spouse to help you. Make sure you have looked into all your options. Don't be too hasty with that trip to Switzerland. My thoughts are with you.
