    Dementia will eventually damage her ability to go to the bathroom on her own. Incontinence is where a lot of people draw the line, but by that point it will be very stressful on the caretaker to make any major decisions, so it is best to handle big moves before reaching that stage. Your LO, regardless of her personality prior to being diagnosed, is no longer able to make big decisions. She is no longer a competent adult and cannot have logical conversations anymore. You can try framing things like, "I know you don't need a facility, but I'm going to need it," but it may just be easier not to discuss this with her at all, especially if she does not remember these conversations. She sounds past the point of being able to express her wishes. There are services that can help describe your options. An elder care lawyer can tell you what your options are. You may have senior advisors in your area as well. There are things like respite care and adult daycare which can give you a break if you just need a few hours to yourself. Your GP may be able to recommend some of these local services to you. If you are considering moving closer to your children, you should probably discuss this with them. They may have some familiarity with facilities in their area or know someone else who does. The neurologist adjusting her medication is more to help you than it is to help her at this point, as dementia is incurable. My LO (who likely has LBD) eventually developed bouts of insomnia and would occasionally become violent. We needed medication to sort that out, and even then it's really only half-sorted as he still occasionally cannot sleep. You can discuss her unwillingness to get a checkup with the neurologist, but changes to her medications will likely require her to go in for an appointment.

    First let me say how sorry I am that you and your wife are going through this. A couple of suggestions: Regarding neurologist appointments. The doctor can help with medications. What meds will work changes as the disease progresses and also varies from person to person. My husband’s neurologist also had a social worker from Alzheimer’s Association at the appointments to let me know of resources etc. Regarding assisted living/memory care etc. You need to tour them but really your wife does not. If the time comes that you feel she needs placement she will not recall if she visited that facility before. This is a horrible disease.

    My opinion, based on everything I've read, is that no single solution/treatment has shown to cure or stop the progression of AD because the origin is multi-faceted, and I'm believing the cause starts outside the brain. There is evidence to back that up. That said, my wife (55) was diagnosed with EOA a year ago. Her symptoms started before that. Her neurologist denied her the amyloid treatment because she believed it was too late and would not benefit my wife. The tests reveal no genetic cause for her condition. So, I'm left to look elsewhere. The doctor is only going to treat symptoms at this point. I firmly believe two major contributions to her condition have been poor sleep habits, and her picky diet (ie. the gut-brain axis). All my efforts have been to diminish those negative influences. So, outside of the usual donepezil and memantine, my wife is on hormone replacement therapy (estradiol patch and progesterone), and I have her on a slew of supplements, we go on near-daily walks, I have her at the local gym twice a week for workouts (including weight training), and have altered her diet. My best description of her condition is "stable", based on my observations. But, I've also seen a couple of things (very recently) that give me some encouragement. She seems more alert, and is putting on makeup (something she hasn't paid attention to in awhile). Whether I'm seeing things I want to see, and if it's related to everything we're doing, or the fact that I increased her Lithium Orotate dose a couple of weeks ago, I can't say for sure. It could be all of the above.

    Thats awesome, I give my dad usually 6-9mg of melatonin as well. I need to get a better neurologist, maybe I need to go to the hospital ER instead. The current neurologist just prescribed seroquel, and then I had to beg for a dose increase and then she said nothing else can be done until she further evaluates my dad in 4 weeks. I guess she doesnt want to be liable for anything, but the doctors at big hospitals seem much smarter and might give me the meds that are needed.     Agent for incapacitated person - My mother has Alzheimer’s and was moved into a memory care facility within the last month. Her primary care doctor has been seeing her for years and has documented the progression of her disease. She also has not been managing her finances for years. My father has a durable power of attorney (we are in Texas) and we are trying to make him the agent for her IRA through Vanguard by filling out all the necessary paperwork. Vanguard wants us to send a form declaring her an incapacitated person. However, we are having trouble getting a doctor to sign off on this. Her neurologist refuses to do so and says she needs to go to a psychiatrist. She has never seen a psychiatrist. Even if we did try to get an appointment, it would be December at the earliest. Her primary doctor is saying they won’t sign it either. I am not sure why, as it is very evident she is incapable of managing her money. What do you do in this situation? We need to be able to access her IRA money to pay for her memory care. Please help.
