Risperidone was a game changer for my mom’s agitation and paranoia. She also takes Zoloft.

Unfortunately, the paranoia doesn’t really ever seem to go away. It just gets a little quieter, a little softer. Mine is in the stage where she’s damn near bedridden and she’ll sometimes vaguely talk about how she needs to go home, or go to the hotel, get on the bus, etc.. She is currently on: 10 mg Haldol at 12 PM, Ativan at 4 PM, Depakote, prazosin, trazodone, and another Ativan at 8 PM to help her sleep. It has helped tremendously. Unfortunately, it’s also a lot of sedation. She still able, for the most part, to interact. But those pills are what keeps the aggravation and paranoia where it is now. It took us a long time to get there and a lot of different med options. Good luck.

Was anyone able to successfully stop paranoia or agitation in their LO using meds? - My dad is currently using seroquel because I told the doc that he was very paranoid and aggressive sometimes. I think the doc mostly focused on the aggressive part and prescribed the seroquel as a sedative to calm him down. I know seroquel is used to stop paranoia as well but thats usually in higher doses that an Alzheimers patient probably cant handle. When my dad does become sedated he usually becomes very sad and depressed and cries that he wants to go home even though he is home. Ive been looking into zoloft (setraline) to maybe boost his mood a bit. And maybe that can possibly help with him always being so agitated all the time from his paranoia. Just wondering if anyone had experience controlling these behaviors with meds, not looking for dose info. My dad is physically still able to do most things, hes only 60. The issue is that his paranoia, agitation, and sometimes aggressiveness is becoming unbearable.

Thank you so much for your response, facing the truth is so hard with this disease. I know at some point he will need to be in memory care, once hes too far gone, doesnt remember anyone, or cant clean himself than I would understand. Right now he is starting to get into a child like state sometimes. He feels like crying sometimes and needs reassurance that I love him and sometimes hes very paranoid and aggressive. He would be crying and extremely sad if I left him alone in a MC, im not ready for that yet, I still have more care left to give for my dad. He can still move around well and he can shower when I tell him too. My mom wants him out of the house asap because she has no patience for any of his paranoia. I hope I can keep him home as long as I can and make great memories with him. 🙏❤️

I am so so sorry. It is a tough day. I think you're thinking of Switzerland unless you were just going to go eat delicious baked goods and meatballs until you pop, but they are not a free-for-all of cognitive failure euthanasia). You will want to start talking to experts immediately if you're structuring your plans around that expectation. There are apparently some private services in the US that will handle the preliminaries and visas and arrangements, and hopefully also will be very honest up front before too many checks get written that you probably won't qualify. And you may be an attorney, but if you are not an estate/family or elder law attorney you need to speak to one. They know all the stuff, especially whatever local resources you need to know about, good or bad. While you are working through legal arrangements, don't solely focus on what happens to her when you are going or gone - you also need to consider if something happens to her before then. When you start getting angry, they will put you on antipsychotics. You can't divert because it's the brain damage, not a choice you'll be making. Not everyone gets like that, either, it really depends on what parts of the brain are affected. Some people stay pretty mellow but the seroquel also helps with your sleep falling apart, hallucinations, paranoia, anxiety. If you don't already have a meditation practice, it can't hurt to try to develop enough of one that you can remember to keep using it for a while. I have a friend who's an Occupational Therapist who works primarily with TBI patients, and one of the things she works on with them/their caregivers is figuring out what kind of sensory stimuli tends to calm your nervous system when it is agitated, and I feel like more people in early-onset especially should pursue that kind of assessment/training. You may be able to continue self-regulating or co-regulating with help for longer than people who haven't worked on that. Give your wife a few weeks and therapy appointments to just process the blow, and then start having some real talks together about where you draw the various lines - driving, being home alone, her needing help with household tasks because more of her energy is on you, when you can't make sound enough decisions to handle money, when security becomes an issue. Not all your wishes will be actionable, but I do feel that having family who knows your general advanced preferences struggle less with decision-making as crises occur. You might also talk about anything you'd like to do together while you can. Friends of mine who have had that opportunity (and it's been with cancer too, not just cognitive impairment) really cherish those memories, even though it can be hard in the moment to JUST be in the moment. You may eventually need to go into care for her safety and wellbeing. I would ask you this: please don't make her swear to never "put you away". After what I've been through, I would never ever make another person swear to sacrifice themselves to being elbows-deep in my poop and insomnia and paranoias and unsafe behavior 24/7. On the flipside, my mother's occasional demands that I kill her or go leave her somewhere are...unsettling. That is not who I am as a person, and I strongly dislike being assumed to be someone inclined to commit crimes or even neglect.
