I have done it in both directions a few times. Being doing this since before there was a word for it. And likely do it again next year. All I say bellow depends a lot on how old are. I look at it on cashflow basis. I also never assume that nomading will net me savings on expenses. In fact I assume the opposite and limit my trips accordantly. What I usually do is make a list of destination and track costs. Then answer: Is my independent income enough to cover all my current expenses plus destination? (I include visas, lawyer, insurance…) Usually if the answer is no I search for a destination where the answer is yes or reduce at home expenses the equation destination plus home are at brake even plus incidentals. If I can’t math both things then I do not do it.

That's a rough situation and honestly pretty common for early retirees with taxable brokerage accounts. Cap gains don't care about your cash flow, they just show up in MAGI and blow the whole thing up. The $20 to $750 jump is exactly what makes the cliff so punishing. It's not a tax, it's a wall. On the IRA question, you can contribute to both a traditional IRA and a Roth IRA but the $7k limit is combined across all IRAs, not per account. So that doesn't give you much runway when you're $20k+ over the cliff. The math you're doing, paying out of pocket and growing the portfolio instead, actually makes sense in your specific situation. $750 a month plus a $12k deductible is $21k before insurance pays a dollar. If you're generally healthy and have the cash reserves to absorb a bad year, self-insuring is a legitimate strategy. The cap gains timing problem is the hard part. Unless you can shift gains to a lower income year or find losses to harvest against them, the cliff is just a cost of having a working trading systrem.

Everyone knows about it. How could you be self-employed and paying for insurance and not know about it?

In France a landlord cannot get insurance for unpaid rent if your tenant does not make 3x the rent in net income. So they are quite strict on this. As I'm sure many other countries are as well.
